What is inside
The template has 11 columns built around the documentation an OCR investigator would ask for: who was trained, when, on which content, with what score, with proof of attestation. The "Days to Refresh" column is a formula so overdue training surfaces immediately.
The columns
- Employee name + Role — Every workforce member — employees, volunteers, trainees, contractors under your direct control.
- Hire date — For new-hire training timing. Workforce must be trained within a reasonable period of joining.
- Last HIPAA training date — The most recent completion. The basis for the annual refresh calculation.
- Course provider — Internal, TeachMeHIPAA, Medcurity, RCMTask, etc. Useful for the audit trail.
- Quiz score — OCR sometimes asks for proof of comprehension, not just attendance.
- Certificate URL / file — Where the PDF lives. Cloud link or filename pointer.
- Attestation signed date — When the employee signed that they understand and will follow the policies.
- Next refresh due — One year from the last completion. Calculated automatically.
- Days to refresh + Status — Countdown formula + plain-English status (Current / Due soon / Overdue / Pending).
How to use it
- Download the CSV. Open in Excel or Google Sheets.
- Add every workforce member: employees, volunteers, trainees, contractors. Part-time staff count.
- Sort by "Days to Refresh" ascending. Anything at zero or negative is overdue training that needs to be assigned now.
- Retain the register for 6 years per HIPAA. Do not overwrite — copy to a new sheet each year and archive.
- When the spreadsheet stops working (volume, version-chain tracking, audit-traceable attestation), graduate to RCMTask's HIPAA training module — $1 per test through August 31, 2026.
